How Do I Switch to Hume Pharmacy?

  • Call or stop by the pharmacy to set up a new patient profile (Personal Info, Phone, Address, Insurance etc.).
  • Provide a list of medications and name/phone number of current pharmacy for pharmacist to transfer prescriptions.
  • Let us know when you need your next medications filled and if you would like to have our med sync service.
  • Don't forget to let us know any specific information about you that helps us personalize your care!
